All MICS student-athletes MUST have an approved NCHSAA SPORTS PHYSICAL (to be completed by a physician) and a MICS MINOR LIABILITY WAIVER in order to be eligible to participate in off season workouts, try outs, and participation in a Fall, Winter or Spring sport prior to the first day of tryouts, no exceptions. If a player has played a previous sport, the player must make sure they have a valid NCHSAA sports physical and minor liability waiver prior to the first day of tryouts/workouts. Both forms are attached to this post. ALL FORMS MUST BE COMPLETE, THIS INCLUDES STUDENT SIGNATURES ON THE FORMS, all boxes checked and questions answered.
Physicals and waivers are good for one full year (395 days). For example, if your player has had a NCHSAA physical completed on August 30, 2019, it is good until September 30, 2020. (Boy Scouts, Little League, CMS, or Well Check Exams are not accepted in place of an NCHSAA sports physical), you must turn in the NCHSAA physical and MICS Liability waiver. Please do not include immunization forms.
You may send in paperwork thru the front offices, drop by Ms Bennett's office (D gym) to hand deliver, or fax to 704.827.8675. Please do not email forms or turn in to coaches the day of tryouts. All forms must be turned in to Elementary office, MS office, or Ms Bennett's office no later than 1:00 pm on tryout day. Please keep in mind there will be close 200 kids trying out for spring sports and many tryouts are at the same time, so paperwork needs to be in by 1:00 pm to Ms Bennett to be recorded for the coaching staff to reference at their facility. Coaches and teachers cannot accept paperwork.
Mandatory Parent Meeting - A parent meeting presentation will be sent out via Parent Square after rosters are submitted by Coach Wylie.
